Dental fee system and clinical application of longterm care insurance in Japan

Abstract
Longterm care insurance started from July of 2008. Longterm care insurance users are 290,000 as of July of 2012 and
37.5% are using nursing home. Elders in nursing homes have poor oral health compared to elders not in nursing home. Longer they stay in nursing home, elders had poorer oral health. Therefore, the needs of dental treatment for elders in nursing home are high. However, Longterm care insurance rarely covers any of the oral health. Also, part time medical doctor or oriental doctor and full time nurse work in nursing home, but part time dentists or full time dental hygienist does not work in nursing home. Thus, management of oral disease and oral hygiene by professional can not be performed. So, effective oral health service in longterm care insurance should be developed urgently and come up with the insurance fee that service provider, insurer and beneficiary can all agree on. In case of Japan, home-based dental care is covered by medical insurance and dental care in nursing home is covered by longterm care insurance. Japan also has dental fee structure and adding structure in variety of items. Since Japan has advanced development in expenses of longterm care and medical structure, Korea should develop expenses system based on Japan¡¯s case.
